Při něm je pacient při vědomí, dokáže myslet, vnímat, slyšet i rozumět, avšak není schopen pohybu ani komunikace s okolním světem, jelikož všechny vůlí ovlivnitelné svaly jsou paralyzovány. Když Richard Marsh po dvou dnech po mrtvici znovu nabyl vědomí, mohl na nemocničním lůžku jen bezmocně přihlížet, jak se lékaři ptají jeho ženy, zda ho mají odpojit od přístrojů. On však nechtěl zemřít a byl rozhodnutý bojovat. Jeden z lékařů naštěstí usoudil, že Richardova situace možná není úplně beznadějná. Zjištění, že Richard může komunikovat mrkáním, stálo na počátku jeho dlouhé a svízelné cesty k uzdravení. Nikdo přesně neví, jak je možné, že se Richard Marsh uzdravil – 90 % pacientů se syndromem uzamčení umírá během prvního měsíce, syndrom je v podstatě neléčitelný. Richardovi se to však podařilo a v knize Zazděný zaživa popisuje, co všechno během dlouhých měsíců rehabilitace prožíval.
