Advances in heart surgical treatment over the past 15 years have been remarkable, with procedures that were once experimental now routinely performed for end-stage heart disease. Heart transplantation has become a well-established procedure, with the International Society of Heart and Lung Transplantation reporting 40,738 heart transplants by the end of 1997. Survival rates are 78% at one year, 65% at five years, and 42% at ten years. This progress stems from significant efforts to understand and manage immune responses to transplanted hearts, develop strategies to control infections, and enhance outpatient care. As a result, heart transplantation has become integrated into healthcare systems, physician practices, and patient awareness. However, the growing number of potential organ recipients has not been matched by an increase in available donor organs. This discrepancy has led to advancements in mechanical circulatory support systems as a treatment option for critically ill patients with failing hearts. Initially used to support circulation in post-cardiotomy heart failure, these assist devices have evolved since the late 1980s to primarily bridge patients with heart failure until a suitable organ becomes available for transplantation.
