Recent advancements have significantly enhanced our understanding of the endothelium's role in regulating vascular tone and organ perfusion in both health and disease. Research in physiology, pharmacology, and molecular biology has revealed critical insights into the structure and function of this often-overlooked "organ." Clinical medicine now faces the challenge of elucidating the clinical implications of these findings, particularly concerning blood coagulation mechanisms, such as the interaction between platelets and the endothelium, as well as vasomotor control and smooth muscle tone regulation, which affect vascular resistance, conductance, and organ blood flow. Additionally, lipid metabolism and its connection to atherosclerosis remain crucial areas of study. The need for improved therapeutic approaches and the development of new pharmacological strategies must also be addressed. The topic of endothelial mechanisms in vasomotor control, highlighted at the 1990 Gargellen Conference, is relevant to both basic scientists and clinicians. The Society for Cooperation in Medical Science (SCMS) aims to promote collaboration between basic science and clinical research, reflecting two key trends: the integration of basic sciences and their methodologies, alongside the statistical validation of concepts and therapeutic strategies through large-scale population and multicenter studies.
