This was a retrospective study of serum and synovial fluid parameters in septic and aseptic arthritis. Synovial lactate had the highest diagnostic potential for diagnosing culture-verified septic arthritis. The synovial fluid white blood cell count was the inflammatory parameter with the highest diagnostic potential for differing between culture-negative septic arthritis and aseptic arthritis. Therefore synovial lactate should be introduced in daily clinical practice in suspicious joints.
